With over 25 years of experience in sports medicine and outpatient physical therapy, Jon Rhodes, DPT, is dedicated to improving patient outcomes through innovative, evidence-based care. As the founder of Rhodes Physical Therapy in Farmington, UT, Jon established the clinic in 2016 with a vision to integrate cutting-edge technology and state-of-the-art equipment to enhance the rehabilitation process.

Our Values

Jon’s commitment to excellence in physical therapy is reflected not only in his clinic but also in his continuous pursuit of knowledge. He holds a Doctorate of Physical Therapy from Regis University (2009), a Master’s of Physical Therapyfrom the University of Colorado, and a Master’s of Business Administration with a focus on marketing from Troy State University. Jon also earned a Bachelor’s degree in Exercise Physiology from Brigham Young University.

Outside of the clinic, Jon enjoys spending time on the golf course and cherishes moments with his family. He and his wife Angie are proud parents to six amazing children.

Jon Rhodes

Melanie Baker

Melanie’s journey in physical therapy began at Lakeview Hospital when she was just 19, where she felt an immediate connection to helping people heal and thrive. She earned her degree in Physical Therapist Assisting from Salt Lake Community College in 2000, launching her career at the University of Utah Hospital and its Outpatient Spine Center, where she treated a wide range of patients and conditions over several years. She has been with Rhodes PT since 2022.

Henry White

Henry White brings a wealth of experience and dedication to his physical therapy practice. A graduate of the University of Utah’s PT program in 1996, Henry has continued to expand his expertise with advanced training in Fascial Counterstrain, a specialized technique he skillfully incorporates to support his patients’ healing and mobility.

Henry’s love for golf began early, and he became a PGA member in 1987. He still competes in local senior professional tournaments, where his competitive spirit and passion for the sport shine. Outside of physical therapy and golf, Henry cherishes time with his wife, Lisa, their four children, and six grandchildren. He also finds fulfillment in his church and community responsibilities, embracing every opportunity to make a positive impact.

Hallie Faciszewski

Hallie Faciszewski joined Rhodes PT in August 2024, bringing her dedication and expertise to our team. Originally from Savannah, GA, Hallie earned her Bachelor’s degree in Exercise Science from the University of West Florida in 2017 and completed her Doctorate in Physical Therapy at Emory University in 2021. In 2023, she and her husband relocated to Utah when he was stationed at Hill Air Force Base.
Beyond the clinic, Hallie loves to explore new places, attend sporting events, and spend quality time with her family. Her enthusiasm and commitment to helping others make her a valued part of the Rhodes PT family.

Dallin Johnson

Dallin brings a wealth of knowledge and enthusiasm to his work in physical therapy. He earned his Bachelor’s degree in Exercise Science from Utah Valley University, followed by his Doctorate in Physical Therapy from Duke University. Dedicated to enhancing his expertise, Dallin is also a Certified Strength and Conditioning Specialist and has completed the Pre-Script Performance Nutrition certification. His curiosity and commitment to learning keep him at the forefront of the field.

This year, Dallin and his wife, Tasia, welcomed their first child, a beautiful and healthy daughter named Scottie. Raised in Davis County, Dallin enjoys spending time with his family, embracing every moment of parenthood. Growing up, he played basketball, soccer, and ping pong—and he’s always ready for some friendly competition in just about anything.

Ashley Tanner

Ashley Tanner, DPT joined Rhodes Physical Therapy in 2021 and has been a fantastic addition to the team, beloved for her caring demeanor and excellent patient interaction skills. She earned her Doctorate in Physical Therapy from MCPHS University in Worcester, MA, in 2018 and has since pursued advanced certifications in Manual Therapy and Dry Needling to deepen her expertise. Originally from Sandy, Utah, Ashley attended Weber State University on a tennis scholarship, where she graduated with dual Bachelor of Science degrees in Athletic Training and Athletic Therapy. Outside of work, she enjoys staying active through tennis, golf, skiing, and hiking with her dog, Mowgli.

Chandler Flores

Chandler Flores, PTA, recently joined us from Arizona, bringing her passion for the outdoors and expertise in physical therapy to Utah. Growing up in the picturesque White Mountains of Arizona, Chandler went on to earn her undergraduate degree at Northern Arizona University in Flagstaff and later completed her PTA license at Carrington College. With a strong background in orthopedic and sports physical therapy, Chandler has a knack for designing personalized programs focused on functional movement and progressive loading. She especially loves helping people return to sport and guiding first responders back to work safely and confidently.

Now calling Utah home, Chandler enjoys the beautiful mountains and changing seasons with her fiancé and their dog. In her free time, you’ll find her outside—skiing, snowboarding, or riding her downhill bike on adventures across the country and beyond. Her vibrant energy and dedication to helping others make her a valued part of our team!
